Once you’ve purchased admission to one of our available courses, please fill out the form below to complete your registration.
Registration is required for all individuals attending a course! If you purchased multiple seats for any course, you must fill out the form for each individual seat. To register multiple seats, fill out the form to completion, submit it, then fill it out again for each additional seat.

Please note:
- Contact us if your certification has expired or expires prior to course date. We are not responsible for unforeseen recertification problems.
- Attending the Audiometric Testing seminar alone meets OSHA requirements for demonstrating competence in audiometric testing. Attending both the Audiometric Testing and Hearing Conservation seminars meets OSHA requirements for CAOHC training equivalence. This approach may be an option if you don’t need CAOHC certification.
